A Tweet Chat is a virtual meeting or gathering that occurs on twitter. The Healthcare Leaders Tweet Chat (#HCLDR) was initiated as a means to gather an international and inter-disciplinary community of health care leaders who are interested in continuing and sharing their professional development.  The Learning Healthcare System (LHS) has been proposed by the Institute of Medicine to maximize available data and information for incorporation into evidence-based delivery of services.  The tweet chat is a mechanism to foster delivery of knowledge to both providers and patients, addressing a Patient Centered Outcomes Research Institute (PCORI) priority. Importantly, both patients and advocates have fully integrated into #HCLDR community discussions and are considered a critical measure of success.  A blog http://hcldr.wordpress.com/2013/06/10/june-11-chatjackandraka/ provides optional pre-assigned reading, source material and the three questions to be discussed during the upcoming one-hour chat. Examples of discussions to date include; Truth and Candor, The Patient Experience, Conference Tweeting, Medicine and Music, Physician Burnout, etc. This learning environment encourages the community to learn from each other.  The demographics of our chat have continued to become more diverse including more community members from the United Kingdom and one from Singapore. While the first community members consisted mostly of physicians and patients we now have community members that include a hospital ceo, a Chair of Radiology, scientists and music therapists.  
Beginning in Oct, 2012, #HCLDR averages 40 people during the weekly exchange of thoughts and practices.   A leader in healthcare tweeter use analytics, Symplur http://www.symplur.com has recognized this tweetchat as a “highly trending” group, with downstream discussions extending throughout the week, resulting in millions of “impressions”. Community members report the chat has helped with physician burn out, increased patient engagement and advanced provider/patient interaction.   Community members learn practical and current information that are used on a regular basis, mentor relationships have developed and this chat has been recognized during a medical conferences for providing a important novel healthcare services.
